Age 54 of New Boston, died December 21, 2017 at his home He was born on June 12, 1963 in Portsmouth to James and Nancy (Shoemaker) Cook, Sr. and worked in the maintenance department for Community Action and Vinyl Craft In his spare time he enjoyed fishing, hunting, camping, and was a big fan of the Bengals and Buckeye football teams.

He is survived by his wife of 18 years, Diana; mother, Nancy Lockert of Sciotoville; sons, Matthew Cook, James Cook IV, and Shawn Cook, all of Portsmouth; daughters, Patty Forest of Ashland, KY, Lisa Cook of Portsmouth, Tasha Taylor of Portsmouth; stepchildren, Joey and Doug Hunt, both of Portsmouth; brothers, John Cook of Hopkinsville, KY, Albert, Roy, Leslie, and Joey Cook, all of Portsmouth; sisters, Regina Scalf of Sciotoville, Connie Mills of Hopkinsville, KY, Wanda Evans of Portsmouth, Ida Noel of Portsmouth; 26 grandchildren; many nieces and nephews He was preceded in death by his father.

Funeral services will be held 11:00 a.m. Friday, December 29, 2017 at D.W. SWICK-NELSON FUNERAL HOME in New Boston with Pastor Danny Deck officiating Interment will follow at Scioto Furnace Cemetery Friends may call Thursday from 1 – 4 p.m. and Friday one hour prior to the service.
